Location: Farmingdale, New York Category: Nursing / Nursing Leadership Schedule: ['Full Time, with Weekend and Holiday rotation'] Shift: Day shift Hours: 8:30am- 4:30pm Req ID: 74268 Overview Clinical Project Manager – Behavioral Health   Catholic Health Home Care is seeking an experienced and compassionate Clinical Project Manager – Behavioral Health to oversee the daily operations of the agency’s Behavioral Health Home Care Program. This position provides clinical and operational leadership to Behavioral Health Nursing staff while supporting the coordination, quality, safety, and continuity of care for patients with psychiatric and behavioral health needs.   The Manager works collaboratively with clinical staff, system facilities, physicians, patients, families, and other healthcare professionals to facilitate safe and effective transitions of care. This role serves as a key member of the management team and participates in organizational planning, policy development, quality improvement, and decision-making related to behavioral health services.   Job Details Essential Responsibilities Manage the daily operations of the agency’s Behavioral Health Home Care Program. Provide leadership, direction, instruction, and guidance to Behavioral Health Nursing staff and other psychiatric and non-psychiatric professional and non-professional personnel. Assist in the coordination and delivery of safe, effective, and patient-centered care. Liaise with hospitals, healthcare system facilities, and onsite staff to facilitate safe and appropriate discharge planning for patients, as needed. Provide clinical support and guidance to on-call staff regarding after-hours issues involving psychiatric and behavioral health patients. Participate as a member of the management team in organizational planning, policy formulation, program development, and decision-making. Ensure compliance with Catholic Health Home Care policies and procedures and all applicable federal, state, and local laws, regulations, accreditation standards, and clinical requirements. Promote and demonstrate service excellence in all interactions with patients, families, staff, referral sources, and healthcare partners. Provide clinical consultation and support to staff regarding psychiatric and behavioral health concerns that may impact patient safety, treatment adherence, and positive patient outcomes. Participate in employee orientation, competency assessment, field clearance, and ongoing professional development. Monitor and oversee the timely and accurate completion and submission of clinical documentation. Ensure staff adherence to agency documentation requirements, policies and procedures, regulatory standards, and accreditation requirements.   Qualifications Current Registered Nurse (RN) license in the state of New York. Bachelor’s degree in Nursing.  Master’s preferred. Psychiatric/Behavioral Health nursing experience required. Previous clinical leadership, supervisory, or management experience. Home care, community health, hospital discharge planning, or care coordination experience preferred. Strong knowledge of psychiatric nursing principles, behavioral health care, patient safety, and regulatory requirements. Excellent communication, leadership, organizational, and problem-solving skills. Ability to collaborate effectively with interdisciplinary teams and healthcare system partners. Demonstrated ability to exercise sound clinical judgment and manage complex patient situations. Proficiency with electronic medical records and clinical/administrative information systems.   Salary Range USD $130,000.00 - USD $150,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
